Standard nursing care incl. observation, personal cares, bed making, medication admin., care paths, documentation incl. progress notes. On any given day there could be IV transfusion/infusions of blood, platelets, plasma, insulin, Potassium, Magnesium, Iron, Chemotherapy to prepare and administer. IDC insertion/removal. IVC insertion/removal. CVAD monitoring, daily bleeds, dressings. Monitoring of drains e.g. ICC or Ascites. Removal of pigtail drains on occasion. Wound care. Visual observations which can incl. a proximate alarm. Ordering of medications or aids. Preparation of discharge paperwork. Transport of pt's to procedures if they have therapy's insitu. Paperwork associated with transferring of pt's. Preparing pts for procedures. Hypertension observations and preparing them for certain procedures. Education on procedures, wound care, subcut injections upon discharge. Stocking and ordering of ward stock. conversing with Allied Health staff, educators, and doctors/teams. Updating and conversing with family members.

Most of the time, I'm anxious coming to work. I get the same allocation as regular staffs, same number of patients and the same level of patient acuity. Most of the time I feel like an Agency staff, relying on help/guidance from the nurse in charge during the shift or from co-staff if it's not so busy.

Working in HDU with 12 monitored beds, the patients primarily are post-operative and cardiorespiratory patients. The unit also admits patients for ketamine infusions for chronic pain management. Day to day roles include telemetry and cardiac monitoring, personal care, managing deterioration, medication administration/titration, and organising discharges and admissions.

I have a minimum of 5 patients, but often have 6. There is no support. The TL's have their own patient load and are not available to help, there is often no AIN to help, the educator is always busy with her own patient loads so she can never help either. These are sick, heavy patients that need more one on one care that our staffing allows. As a result, the patient's suffer as things are missed and they get minimal care. The nurses are just trying to get everyone their medications and get all the paperwork done, so often the little things that make a huge difference don't get done, such as communication, proper peri care, PAC, skin assessments, taking that extra 3 minutes to explain what's happening or answer their questions/concerns. Also, we don't have enough time to notice subtle signs of deterioration. We are run so hard that we're lucky to get a break on a shift.
